Position: full-time position

SCAN is establishing a project that helps with care coordination for individuals experiencing homelessness and have Severe Mental Illness/Severe Emotional Disturbance within a behavioral health care setting.

Duties: Provide an assertive case-management approach to identified needs of clients. This will include but is not limited to transportation, scheduling of appointments, help participants in attending appointments, advocacy, assistance with accessing public benefits, and enhance community awareness.

Minimum Qualifications:

• High School degree or equivalent
• Must be fluent in both Spanish and English
  
• Good computer skills
  
• Transporting clients in own vehicle is required at times and compensated for mileage
• Reliable transportation, proof of insurance and a valid driver's license required
• Able to work flexible hours and weekends
