Position Description
Position: Part-Time Case Manager (MAT) (29 Hours per week)(Position will end on September 29, 2026)

We are seeking a part-time case manager to work at our Office Based Opiate Treatment Program (OBOT). The OBOT program provides individual and group counseling along with medication assisted therapy using Suboxone.
WORK SCHEDULE
The schedule for this position will be Tuesday-Thursday 8:00 AM to 5:00 PM. This schedule is subject to change based on patient's needs.
JOB RESPONSIBILITIES
- Monitor clients progress, including medication adherence (e.g. Suboxone and other psychotropic medications) and participation in required counseling services.
- Collect and document urine analysis (UA) specimens in accordance with program protocols and confidentiality regulations, monitor results and follow up as needed.
- Provide care coordination and referrals to essential services such as housing, mental health support, employment assistance, and/or legal advocacy.
- Educate clients on Medication Assisted Treatment, opioid use recovery, overdose prevention (e.g. naloxone), and supportive intervention strategies.
- Maintain accurate and timely documentation of assessments, treatment plans, progress notes, UA results, and referrals in compliance with HIPAA and organizational policies.
- Conduct regular client check-ins to support treatment engagement and address barriers such as transportation, stigma, and/or co-occurring disorders.
- Respond to client crisis, including relapses, withdrawal symptoms, or mental health emergencies, and connect clients with appropriate services.
- Facilitate discharge and aftercare planning to support long-term recovery, including connection to peer support, outpatient care, and recovery housing.
- Participate in multidisciplinary team meetings, case reviews, and ongoing training related to opioid addiction treatment and recovery support.
MINIMAL QUALIFICATIONS
- Associate's degree in human services, Social Work, Psychology, or related field
- At least 2 years of relevant experience in case management, substance use treatment, or behavioral health.
- Valid driver's license and reliable transportation.
KNOWLEDGE, SKILLS AND ABILITIES REQUIRED- Knowledge of opioid use disorder and Medication-Assisted Treatment (MAT) services
- Ability to collect and handle urine analysis (UA) samples per program protocols
- Strong organizational and communication skills
- Familiarity with trauma-informed, client-centered, and recovery-oriented care
- Basic computer skills and ability to use electronic health record (EHR) systems

Salary: $14.00 to $15.00 per hour depending on experience and qualifications.
